What probably happened in the last one with the blunderbus accident was a hangfire. The percussion cap went off, but the flame jet didn't reach far enough into the chamber of the gun so it didn't go off immediately. However, the cap left some burning ember inside the channel leading from the cap to the weapon chamber, and when the bloke who tried to fire the blunderbus tried to get the weapon back into the car, some of the black powder managed to pour into that channel, come into contact with the still-glowing ember, and set off the charge while the business end of the barrel was pointing into the sedan.

For the cannon one, these small cannons usually have a small bit of fast burning rope to light them, a but of that probably flew up from the touch hole and by pure chance fell into that BP container.

Regarding safeties, most hand gun users do not use them, and in fact the Glock models do not have one. The safety is don't pull the dam trigger lol
